Data change request

A patient's personal details and contact information are very important to ensure communication between the patient and SJD Barcelona Children's Hospital. That is why any incorrect information must always be amended.

Fill in the Data change request form to update the contact details (change of address, telephone, etc.) that are listed in a patient's clinical record at SJD Barcelona Children's Hospital.

We also recommend that you report this change to your primary care centre (CatSalut). In the event that the details listed in the SJD Barcelona Children's Hospital medical record do not match those of the CatSalut record, those recorded by CatSalut may be referred to.

You can change these details at any CatSalut centre.
